GENERAL DESCRIPTION:

JOB SUMMARY:
The Licensed Clinical Social Worker reports to the Behavioral Health Director. Under general supervision and with latitude for independent judgment. The Licensed Clinical Social Worker is a part of the person centered integrated care team and provides direct and indirect services to persons served in both the primary care and behavioral health setting.

Full-Time Medical Scribe/Certified Medical Assistant position opened for our busy Doniphan Urgent Care located at 109 Plum Street Doniphan, MO

Job description:
Main position will be for the medical scribe but fill in as a Certified Medical Assistant as needed.
A scribe’s core responsibility is to capture accurate and detailed documentation (handwritten, electronic, or otherwise) of the encounter in a timely manner. 
Scribes are not permitted to make independent decisions or translations while capturing or entering information into the health record or EHR beyond what is directed by the provider. 
Scribes are responsible for capturing medical information at the point of care which allows the provider to focus on bedside manner and provide hands-on, attentive, face-to-face care that increases both patient and provider satisfaction. 
 
The general duties of a scribe may vary and can include:
  • The Medical Scribe will assist the provider in navigating the EHR.
  • The Medical Scribe must be able to locate information for review (i.e., previous notes, reports, test results, and laboratory results)
  • The Medical Scribe must respond to various messages as directed by the provider.
  • The Medical Scribe will enter information into the EHR as directed by the provider.
  • The Medical Scribe will research information requested by the provider.

QUALIFICATIONS

  • The Medical Scribe must have high school education or GED equivalent.

A Nurse Care Manager is an integral part of the Patient Centered Medical Home Team with their primary role being to coordinate and manage the care of vulnerable patients.

RESPONSIBILITIES:

  • Maintains current CPR Certification Must be willing to travel.

The Referral Coordinator/Case Manager works as part of the medical office team performing administrative and clinical duties necessary to arrange appointments for specialty care for patients outside of MHHC.

 Duties include gathering patient information, calling outside specialists/hospitals/offices to schedule and complete referral, faxing and mailing information to consulting facilities, and notifying patients of their appointments. 

Responsibilities 

· Ensure patient confidentiality and familiarization of HIPAA laws.

 · Process patient information and chart notes for referrals to other Health Care facilities. 

· Contacting insurance companies for prior authorization for referrals. 

· Tracking the process of the referrals and completion of the referrals.

The Risk Assistant reports to the Risk Manager. The Risk Assistant is responsible for leading patient safety/clinical risk management initiatives impacting clinical and operational processes throughout MHHC. The Risk Assistant’s specific focus will be toward the review and reduction of medical errors and other factors that contribute to unintended adverse patient outcomes and patient dissatisfaction. JOB OBJECTIVES:

  • Use clinical expertise to identify and evaluate activities or events occurring within the Health Center that have the potential to lead to patient injury and/or which represents opportunities to improve the safety of services offered by the Health Center.
  • Provide effective leadership for conducting patient safety/clinical risk assessments and investigations, including root cause analyses and implementation of effective corrective action plans.
  • Support and assist in the implementation of performance improvement plan initiatives arising out of root cause analyses and clinical risk management investigations.
  • Demonstrates clinical and technical expertise in the areas of patient safety and clinical risk management and provide consultative assistance to individuals or groups as necessary.

Directs entity’s safety programs to protect employees against harm, and maintain safe working conditions.
