Missouri Highlands Health Care is seeking a full time Family Nurse Practitioner to FLOAT throughout our 10 Medical Clinics.

Travel is required. Mileage is reimbursed.

FUNCTIONS AND RESPONSIBILITIES:

• Provides clinical management for clinic patients on site or off site, including health supervision of uncomplicated illness, including chronic illness, persons with minor accidents and anticipatory guidance.

• Elicits and records health histories for individuals and families in a systematic and succinct form and critically evaluates and documents the findings in the patient’s chart.

• Assumes responsibility for health surveillance relevant to health teaching to promote optimal physical and mental health to prevent illness and establish a Wellness mode.

• Must show proficiency in computer skills, as MHHC utilizes EMR exclusively for patient charting and billing.

• Supervises, as deemed necessary, allied health personnel when appropriate to complete above objectives.

• Uses sound judgment in deciding which persons can be served by the Nurse Practitioner and which should be referred either to physicians for differential medical diagnosis or to other community resources.

• Collaborates with other professional staff members, physicians and other health professionals in devising improved and ongoing ways to deliver quality primary health care.

• Participates actively in development, approval and implementation of sound protocols under which the Nurse Practitioners function. Follows said protocols when providing health care services to individuals seeking care.

• Participates in CME in order to say abreast of current trends in health care delivery, expansion of personal skills ad knowledge and enhancement of services provided through the organization. Outreach and Enrollment Coordinator assists consumers with enrollment and navigation of health plans, develop relationships with health plan service providers, manage monthly reporting and facilitate health care and social service resources to support the health and well being of the organizations clients and potential clients, and educate and network with people of diverse backgrounds and cultures.

Duties:

  • -Manages services and assist consumers with enrolling in health plan offerings and others services, sample program(s) and/or service initiatives.
  • -Tracks enrollment and outreach activities outcomes and provide monthly reports.
  • -Ensures routine adherence to all regulations surrounding HRSA, CMS, MPCA, etc., as well as all laws and regulatory requirements surrounding confidentiality.
  • -Makes outreach calls to health plan members.
  • -Organizes and attend community events and health fairs as assigned.
  • -Raises organizational service awareness in the community.
  • -Reports activities and challenges to PCHH Director in written format for health plan documentation requirements, reporting unusual or significant occurrences in a timely manner.
  • -Bridges communications and information between health plan members and health care delivery systems.

Requirements:

  • Must be able to type at least 30 words per minute
  • Must be able to use office equipment (i.e. telephones, and fax machines)
  • Must be able to use Microsoft software

Qualifications:

  • Education: High School Diploma or GED required. Bachelor’s Degree preferred. Experience: 1-2 years Customer Service Experience required. Experience working in a clinical/medical environment is preferred.
  • Other: Must have Reliable Transportation and be personally insured. Must be able to travel.

The Referral Coordinator/Case Manager works as part of the medical office team performing administrative and clinical duties necessary to arrange appointments for specialty care for patients outside of MHHC.

Duties include gathering patient information, calling outside specialists/hospitals/offices to schedule and complete referral, faxing and mailing information to consulting facilities, and notifying patients of their appointments.

Responsibilities

· Ensure patient confidentiality and familiarization of HIPAA laws.

· Process patient information and chart notes for referrals to other Health Care facilities.

· Contacting insurance companies for prior authorization for referrals.

· Tracking the process of the referrals and completion of the referrals.

· Working with other facilities on arranging appointments for patients.

· Faxing and/or mailing patient information to other facilities.

· Updating referring provider on status of referral i.e. delay, insurance problems etc.

· Updating patient on status of referral i.e. appointments, delay, insurance problems etc.

· Work cooperatively and supportively with all other staff.

· Perform other necessary duties as required by Missouri Highlands Health Care to achieve the goal of providing quality primary health care services.

Qualifications:

· Previous experience in health care setting is preferred, ideally in an office practice.

· Basic familiarity with routine medical protocol and terminology.

· Must be able to complete Basic Life Support training class.

Skills in communication and an affable attitude in order to accept directions, accept constructive criticism, and work with all personnel and patients.

Missouri Highlands Health Care is seeking a LCSW (Licensed Clinical Social Worker) for our Poplar Bluff, MO locations.

JOB SUMMARY:
The Licensed Clinical Social Worker reports to the Behavioral Health Director. Under general supervision and with latitude for independent judgment. The Licensed Clinical Social Worker is a part of the person centered integrated care team and provides direct and indirect services to persons served in both the primary care and behavioral health setting.

JOB OBJECTIVES:
Work cooperatively with MHHC’s providers and personnel to carry out the goals and objectives of MHHC, according to established policies and procedures.
Will treat others respectfully whether it is a patient, fellow employee, member of management, customer, vendor or visitor to our premises.
Assess, diagnose, and evaluate clinical symptoms, developmental and personality disorders, severity of psychosocial stressors and global functioning and address medical conditions as they affect patients’ functioning.
Plan treatment based on the individual client’s needs and resources and on clinic and community resources.
Coordinate professionals involved in patient care to insure interdisciplinary collaboration and continuity of services.
Provide comprehensive and current knowledge of community resources for supplementary treatment of clients and make referrals, which adequately and appropriately utilize these resources. Is available for consultation with other clinic staff regarding community resources.
Provide a continuum of short- and long-term psychotherapy services to individuals, couples, families, and groups, based on individualized treatment plans.
Function as an integral part of a multidisciplinary department including psychiatrists, psychologists and clinical nurses.
Maintain good communication and collaborative relationships with referring physicians and other staff and recommend additional referrals for patients as needed.
Work with collateral personnel as needed in the planning and delivery of services and case management.
Carry out ongoing record keeping and case review of all psychotherapy work in compliance with State guidelines.
Carry out all above-mentioned duties with the highest professional standards, including maintenance of patient confidentiality.
Provide assessment (by telephone and in person) of dangerousness and immediacy of crisis situations.
Design and carry out appropriate interventions in crisis situations.
Offer in-service education programs and functions as a resource to the medical staff and the community as needed.
Participate in development and selection of patient education materials and services.
Represent the Health Center and the Counseling Staff to the community at large in a professional and helpful manner.
Maintain pertinent patient information in the medical record.
Complete the necessary written information and data in a timely fashion.
Must comply with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
Attend staff meetings as requested.
Perform related work as required.

ESSENTAL JOB FUNCTIONS:
Assess and treat a wide variety of patients/clients with accuracy, proficiency, and efficiency at an advanced professional level.
Cognitively read and interpret a variety of instructions furnished in written or oral form.
Effectively present information and respond to questions from patients, and other individuals encountered in the course of employment.
Communicate with patients and office staff with tact and patience utilizing good communication skills.

CRITICAL SKILLS/EXPERTISE:
Master’s Degree in a mental health field with documented clinical course work and field experience. Possess broad background of training and experience with various patient populations and modes of service delivery. Must demonstrate the ability to turn on the computer and the monitor. Must demonstrate the ability to navigate from the computer desktop to Microsoft (MS) Office. Must demonstrate the ability to open, minimize, and maximize different windows in the Microsoft Office applications (see #4 above).Must be able to talk, hear, react, stand, kneel and lift.

Missouri Highlands Health Care is seeking a Dental Assistant for our clinic in Pilot Knob, MO. Dental Assistants perform a number of duties in a dentist’s office. Some tasks may be clerical. Dental assistants with office duties schedule appointments, keep records, receive payments from patients, and order supplies. When patients come to the office, dental assistants locate their medical records for the dentist’s use. II. RESPONSIBILITIES A. Prepare patients for the dentist’s examination. B. Perform chair-side duties, such as handing the dentist the proper materials and tools. C. Operate the suction hose that keeps the patient’s mouth dry so the dentist can work on it. D. Operate X-ray machines. E. Make an impression of a patient’s mouth or teeth. F. Sterilize instruments, develop X-rays, and mix compounds for cleaning or filling teeth. G. Community Health Worker (CHW) roles are designed to improve access to care, increase knowledge, prevent disease, and improve select health outcomes for populations. Community Health Worker’s activities are tailored to meet the unique needs of their communities. CHW’s will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
