Triage Nurse- Registered Nurse – Non-Exempt
JOB DESCRIPTION
POSITION IDENTIFICATION
The Triage Nurse (RN) provides clinical assessment and care coordination primarily through telephone-based triage for primary care and pediatric patients in a clinic environment. This role supports patient access by providing safe, timely telephone triage services that assess patient needs and route the patients to the appropriate level of care.
RESPONSIBILITIES
- Serve as the primary clinical contact for incoming patient and caregiver calls requiring nursing assessment and guidance.
- Perform telephone triage for adult and pediatric patients using triage protocols.
- Assess symptoms, acuity, and developmental considerations to determine appropriate level of care.
- Provide nursing advice, anticipatory guidance, and patient/caregiver education within scope of practice.
- Identify and escalate urgent or emergent conditions to providers or emergency services as appropriate.
- Coordinate care including appointment scheduling, referrals, diagnostic testing, and follow-up care.
- Document all patient interactions accurately and timely in the electronic health record (EHR).
- Collaborate with providers, medical assistants, care coordinators, and leadership teams to ensure continuity of care and care requirements.
- Adhere to infection prevention practices, clinic policies, regulatory requirements, and nursing standards of care.
- May provide direct patient care in the clinic setting when staffing or operational needs arise
- Maintain patient confidentiality and comply with HIPAA and organizational privacy standards.
- Perform other duties as assigned
REQUIREMENTS
- Current, active Registered Nurse (RN) license in the state of Missouri.
- Basic Life Support (BLS) certification required.
- Minimum of 1–3 years of nursing experience in ambulatory care, primary care, and/or pediatrics.
- Pediatric nursing experience preferred, including experience communicating with parents and caregivers.
- Experience with telephone triage and standardized triage protocols preferred.
- Comfortable providing nursing advice and patient education
- Strong assessment, critical thinking, and prioritization skills across adult and pediatric populations.
- Excellent verbal and written communication skills.
- Proficiency with electronic health records (EHR) and clinical documentation.
