Location: Eminence or Ellington Missouri
JOB SUMMARY:
The Licensed Clinical Social Worker/Licensed Masters Social Worker reports to the Behavioral Health Director. Under general supervision and with latitude for independent judgment. The Licensed Clinical Social Worker is a part of the person-centered integrated care team and provides direct and indirect services to persons served in both the primary care and behavioral health setting.
JOB OBJECTIVES:
- Work cooperatively with MHHC’s providers and personnel to carry out the goals and objectives of MHHC, according to established policies and procedures.
- Will treat others respectfully whether it is a patient, fellow employee, member of management, customer, vendor or visitor to our premises.
- Complete comprehensive clinical assessments when appropriate to level of care.
- Assess, diagnose, and evaluate clinical symptoms, developmental and personality disorders, severity of psychosocial stressors and global functioning and address medical conditions as they affect patients’ functioning.
- Plan treatment based on the individual client’s needs and resources and on clinic and community resources.
- Coordinate professionals involved in patient care to ensure interdisciplinary collaboration and continuity of services.
- Provide comprehensive and current knowledge of community resources for supplementary treatment of clients and make referrals, which adequately and appropriately utilize these resources. Is available for consultation with other clinic staff regarding community resources.
- Provide a continuum of short- and long-term individual, family, couples and/or group therapy based on individualized treatment plans.
- Function as an integral part of an integrated, multidisciplinary team including primary care providers, social workers, psychiatrists, psychologist, community health workers and clinical nurses.
- Maintain good communication and collaborative relationships with referring physicians and other staff and recommend additional referrals for patients as needed.
- Work with collateral personnel as needed in the planning and delivery of services and case management.
- Carry out ongoing record keeping and case review of all behavioral health documentation in compliance with CMS and other health insurance guidelines.
- Provide assessment (by telephone, phone and/or in person) of dangerousness and immediacy of crisis situations.
- Design and carry out appropriate interventions in crisis situations.
- Offer in-service education programs and functions as a resource to the medical staff and the community as needed.
- Participate in development and selection of patient education materials and services.
- Perform the as needed role of Behavioral Health Consultant (BHC), as a part of the primary care treatment team, to provide short-term behavioral interventions to patients with medical and behavioral health problems.
- Represent the Health Center and the Counseling Staff to the community at large in a professional and helpful manner.
- Maintain pertinent patient information in the medical record.
- Complete the necessary written information and data in a timely fashion.
- Must comply with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
- Attend staff meetings as requested.
- Perform related work as required.
- Carry out all above-mentioned duties with the highest professional standards, including maintenance of patient confidentiality.
ESSENTAL JOB FUNCTIONS:
- Assess and treat a wide variety of patients/clients with accuracy, proficiency, and efficiency at an advanced professional level.
- Cognitively read and interpret a variety of instructions furnished in written or oral form.
- Effectively present information and respond to questions from patients, and other individuals encountered in the course of employment.
- Communicate with patients and office staff with tact and patience utilizing good communication skills.
- Must comply with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
- Perform effectively as a member of a multidisciplinary team.
- Clean and neat appearance.
CRITICAL SKILLS/EXPERTISE:
- Master’s Degree Social Work with current licensure and documented clinical course work and field experience.
- BLS (Basic Life Support)
- Possess broad background of training and experience with various patient populations and modes of service delivery.
- Must demonstrate the ability to turn on the computer and the monitor.
- Must demonstrate the ability to navigate from the computer desktop to Microsoft (MS) Office.
- Must demonstrate the ability to open, minimize, and maximize different windows in the Microsoft Office applications (see #4 above).
- Must be able to talk, hear, react, stand, kneel and lift.
- Must respect and maintain confidentiality.
