Sliding Fee Scale Discount Program

NOTICE TO PATIENTS:

No patient will be denied care based on their inability to pay for services.

All patients may apply for the Missouri Highlands Health Care’s Sliding Fee Scale Discount Payment Program which is based solely on family size and income.

Please ask the Medical Receptionist for more information and an application for this program. Bring completed application to your first visit along with proof of income and your last tax return.
