Delta Care Coordination

Missouri Highlands Health Care’s Delta Care Coordination Program utilizes community health workers to find the right care for you and help find the community resources you need.

Missouri Highlands Health Care (MHHC) is currently accepting people who have:

Any chronic (ongoing) condition (ex. diabetes, obesity, asthma, anxiety, ADHD, depression, COPD, heart disease, arthritis, etc.)
No Health Insurance

The Program Offers:

An assigned MHHC Community Health Worker (CHW) to help you navigate your chronic condition and ongoing care.
Delta 500 – an allowance of $500 to be used toward prescription medications and other limited medical services.
$25 gas vouchers to needed medical appointments, once per year.

Grocery Store Tours

Shop Smarter. Eat Healthier. Save Money.

Making healthy choices at the grocery store doesn’t have to be confusing or expensive. Missouri Highlands Health Care is now offering Grocery Store Tours led by our Community Health Workers (CHWs) to help you feel more confident while you shop.

During this free program, you’ll meet with a Community Health Worker one on one or in a small group at a participating local grocery store. Together, you’ll learn practical tips that can help you make healthier food choices while staying within your budget.

During your tour, you’ll learn how to:

Choose healthier versions of the foods you and your family enjoy.
Read and understand nutrition labels.
Save money by shopping wisely and avoiding unnecessary purchases.
Reduce food waste through smart meal planning and shopping habits.

Whether you’re managing a chronic condition like diabetes or high blood pressure, trying to eat healthier, or simply looking for ways to make your grocery budget go further, this program can provide practical guidance you can use every time you shop.

How to Participate

Participation in the Grocery Store Tour program requires a referral from your Missouri Highlands Health Care medical provider. If you’re interested, ask your provider if this program is right for you.
