Delta Care Coordination

Missouri Highlands Health Care’s Delta Care Coordination Program utilizes community health workers to find the right care for you and help find the community resources you need.

Missouri Highlands Health Care (MHHC) is currently accepting people who have:

Any chronic (ongoing) condition (ex. diabetes, obesity, asthma, anxiety, ADHD, depression, COPD, heart disease, arthritis, etc.)
No Health Insurance

The Program Offers:

An assigned MHHC Community Health Worker (CHW) to help you navigate your chronic condition and ongoing care.
Delta 250 – an allowance of $250 to be used toward prescription medications and other limited medical services.
$25 gas vouchers to needed medical appointments, once per year.

We Can Assist You With:

Care Coordination of Medical services to Health Departments and other health care providers.
Specialist and Medical Supply Referrals
Understanding and managing your chronic conditions
Understanding doctor recommendations for ongoing care
Bridge the gap between your medical care team and yourself
Basic Screening & Assessments
Health Education and/or Informal Counseling
Advocate for individual and community health needs
Outreach & Engagement
Help locate food pantries and other community resources
