Delta Care Coordination

Missouri Highlands Health Care’s Delta Care Coordination Program utilizes community health workers to find the right care for you and help find the community resources you need.

Missouri Highlands Health Care (MHHC) is currently accepting people who have:

  • Any chronic (ongoing) condition (ex. diabetes, obesity, asthma, anxiety, ADHD, depression,  COPD, heart disease, arthritis, etc.)
  • No Health Insurance

The Program Offers:

  • An assigned MHHC Community Health Worker (CHW) to help you navigate your chronic condition and ongoing care.
  • Delta 500 – an allowance of $500 to be used toward prescription medications and other limited medical services.
  • $25 gas vouchers to needed medical appointments, once per year.
