Welcome to the Missouri Highlands Health Care Foster Care Community Health Worker (CHW) Program!

Our Foster Care CHW is dedicated to serving individuals in the foster care community by advocating for patients’ health needs and addressing social determinants of health. We understand the unique challenges faced by foster families and children, and our goal is to provide comprehensive assistance to support their well-being.

What We Offer

Personalized Support

Our experienced Foster Care CHW acts as a liaison between patients, clinical care teams, and community partners. We work closely with you to identify your specific needs and connect you with the appropriate community resources.

Holistic Care

We assist in identifying and addressing various social determinants of health, including access to food, clothing, transportation, child care, clinical healthcare education, housing, and more.

Comprehensive Services

Our focus is on providing comprehensive support for foster families and children. This includes facilitating access to medical care, behavioral health services, dental referrals, and other essential resources.

Who We Serve:

Our Foster Care CHW program caters to a diverse range of individuals within the foster care community, including:

Children in the care and custody of the DSS (Foster Children/Placement)
Children or Youth in Alternative Care (Children residing with anyone other than biological parents)
Children receiving Adoption or Legal Guardianship Subsidy (Adoption/Kinship/Placement, e.g., grandparents caring for grandchildren)
Former Foster Care Youth under the age of 26 who were in Foster Care on their 18th birthday (We provide support in healthcare education, budgeting, financial education, job applications, and connection to community resources.)
Youth involved in Youth Services/Juvenile Justice (e.g., Youth Ranches and Homes)
