Missouri Highlands Health Care is seeking a Community Health Worker for Poplar Bluff, MO.
Community Health Worker (CHW) roles in communities vary and depend on the sector in which they work social services or health care. These programs are designed to improve access to care, increase knowledge, prevent disease, and improve select health outcomes for populations. Community Health Worker’s activities are tailored to meet the unique needs of their communities. CHW’s will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
RESPONSIBILITIES:
- Assuring that people access the coverage and services they need
- Creating more effective linkages between vulnerable populations and the health care system;
- Managing care and care transitions for vulnerable populations;
- Ensuring cultural competence among health care professionals serving vulnerable populations;
- Providing culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition, and cultural competence;
- Advocating for underserved individuals to receive appropriate services;
- Providing informal counseling , such as social support, care coordination and health screenings
- Building community capacity to address health issues.
- CHWs offer interpretation and translation services to provide culturally appropriate health education and information to assist people in receiving the care they need.
- CHW’s give informal counseling and guidance on health behaviors
- CHW’s advocate for individual and community health needs
- Perform other duties, as assigned.
- Working with our community schools and head starts
- Educate Women and child and minorities on the Medicaid program in all 7 counties we serve
- Focus on trying to sign our patients up for the Medicaid programs
- Provide education and resources to women and children
SKILLS REQUIRED:
- Excellent presentation performance skills
- Clear and concise communication skills
- Cultural competence of the areas they are serving
- Possess patience and professionalism
REQUIREMENTS:
- Any combination of 3 years health/social services experience and/or education
- Verifiable good driving record and reliable transportation
- Background check and fingerprinting
- Completion and passing of Community Health Certification within 6 months of date of hire
